Administrator
Responsible for the overall direction, coordination and evaluation of all facility operating activities. Assures effective planning, organization and implementation of financial, personnel, physical plant, public relations and legal functions of the facility. Responsible for monitoring regulatory compliance to assure high quality of resident care.
Director of Nursing
The Director of Nursing Services is responsible for coordinating the care of the residents who reside within the facility. She directs the clinical team in establishing and enforcing goals for the home and for the individuals we care for. She ensures compliance with healthcare policies and resolves issues as they arise as part of an interdisciplinary team. She maintains close communication with the nursing team, physicians, and consulting providers to promote safe and competent practices.
Assistant Director of Nursing
The Assistant Director of Nursing works closely with the Director of Nursing and other leadership team members to monitor and direct care of the residents. She assists in establishing and enforcing polices and individualized plans of care for each member of our community.
MDS Coordinator
Conducts management data analysis while creating patient care plans to ensure the facility provides consistent quality care that is individualized to meet the needs of each specific person.
Dietary Manager
Responsible for food service production, dining service, planning, organizing, developing and directing the overall operation of the Dietary Department in accordance with current Federal, State and local regulations and Company policies and procedures. Ensure quality food service/nutritional care is always provided for each resident.
Environmental Director
Responsible for ensuring a clean and safe environment for all staff and residents in accordance with Federal, State and local regulations and Company policies and procedures. Oversees all Environmental staff.
Activities Director
Plan, organize and direct the implementation and evaluation of activity programming for the residents. Assure that personalized, individual activity plans are developed and implemented meeting the interest, physical, mental and psychosocial needs of all residents.
BOM/ Medical Records Coordinator
Responsible for coordinating documentation of resident care plans and records by the interdisciplinary team assuring compliance with State and Federal regulations. Will serve as a resource for all disciplines in meeting residents’ needs.
Maintenance Supervisor
Ensures the facility, ground, equipment, furnishings and resident rooms are maintained in a good and safe manner.
Human Resources/ Social Services Director
Performs a variety of administrative and project management services to support the basic Human Resources function at the facility, also provides employees with new hire paperwork: reviews content of new hire paperwork for accuracy and maintain employee files in accordance with State and Federal regulations.
Responsible for the identification of the residents’ cognitive, emotional, mental and psychosocial need and the provision of services to meet the identifies need of the individual to attain or maintain the highest practicable level of physical, mental and psychosocial wellbeing. Also responsible for the business office needs, including insurance verification, collections, and AR tracking.
Restorative Nurse Aide
Assists residents with follow up maintenance of rehabilitation services including, but not limited to, mobility, personal hygiene and feeding.
